Reimbursement of Facility Fee to Attend the MDA ALS Clinic
at the University of Miami

The Kessenich Family MDA ALS Center, under the medical direction of Dr. Walter Bradley, University of Miami, has recently moved their ALS Clinic to another location. The weekly multidisciplinary clinic is now held at:

St. Catherine’s Rehabilitation Hospital
1050 NE 125th Street
North Miami, FL

All ALS patients attending the clinic are now required to pay a facility fee to St. Catherine’s in the amount of $58.00 at the time of service. Private insurance plans may or may not cover the facility fee. Regardless of coverage, LGDA will reimburse its members the cost of the facility fee. To be reimbursed, LGDA members should send their proof of payment to:
